The Simple Marketer contact form is a simple, easy to use contact form that you can include on your webpage. It allows the user to enter their name, email address, phone number, subject, message and select how they want to be contacted back. It also allows the user to select marketing information such as what industry they're involved in, what products/services they're interested in and where they found out about your website. It then sends this information to your email address via PHP.
